ON-SITE CARDIOVASCULAR SCREENING - Lipid profile with immediate results on calculated LDL/HDL, triglycerides and total cholesterol, plus total cholesterol/HDL ratio. Blood pressure, pulse, weight, non-invasive body fat analysis/BMI, and risk assessment also included. Price: $38.00 per person. 4-page personal wellness profile reports provided to individuals for an additional $7.00 per person.

ON-SITE DIABETIC SCREENING - Glucose screening and diabetes risk assessment. Price: $10.00 per person; (includes coupon for free glucose meter for anyone testing out of normal range, up to an $80.00 value)

ON-SITE FLU VACCINE CLINIC - Flu vaccination, plus cold and flu education. Price: $23.00 per person. (Vaccine is still available for the Winter 2007 season)

PANDEMIC FLU PREPAREDNESS - A flu pandemic threatens to disrupt our personal and corporate livelihoods on local, national, and global scales. If your company needs to show preparedness documentation to clients or simply wants to safeguard your business and employees, call us. Pricing variable based on needs.

ON-SITE ADULT CPR AND FIRST AID TRAINING - 4 hours of training for a minimum class size of four to earn certification. Price: $50.00 per person. $7.50 for trainee handbooks. 2-hour AED TRAINING is also available for additional $25.00 per person in conjunction with CPR/First Aid Training.

ON-SITE SMOKING CESSATION - To help your employees kick the habit for good! $1,500/six-week course. Currently provided at NO COST to Elkhart County employers for a limited time.

CASE/DISEASE MANAGEMENT - To educate, advise, and encourage your most "at-risk" employees in a one-on-one approach to achieving optimum health. Service includes review of medical history, treatment and bills, employee consultation, physician referrals, review of job duties and environment, plus discounted rates on certain wellness services. $100 per hour.

EMPLOYEE ASSISTANCE PROGRAM - Referral service for employee depression, mental health, family issues, stress, and drug dependencies. Employees may call On-Site Health Solutions direct for confidential, 24-hour referral. Price: $5.00 per employee per month.

FIRST AID KITS/SUPPLIES - Produced and delivered with periodic maintenance, following customized assessment of your company's specific health and safety needs. Price: Variable based on needs.

ON-SITE LUNCH AND LEARN SEMINARS - A healthy lunch and education for employees on a variety of health-related topics. Can be conducted in conjunction with any of our screenings and clinics. Price per seminar: $250.00 plus the cost of catered lunch.

ON-SITE HEALTH FAIRS - OHS will coordinate an on-site day-long Health Fair with a gathering of representatives from several healthcare associations and providers. On-site assessments, screenings, and lunch can also be integrated at an additional cost. Price: $1,600.00 for set-up and organization.
